Queen Anne’s County Parks & Recreation is looking forward to the opportunity to Discover Summer with your child(ren) through our various Summer Camps. Below you will find the Camp Policy and Forms necessary to help make your son or daughter’s attendance a breeze. Please complete the forms below (one per child in attendance) and bring them with you on the first day of camp.

• Camp Policy, Emergency Form & Authorization to Pick-Up
Read the camp policy and complete all three forms at the end. These completed forms are required for camp attendance. For families with more than one child in attendance, one set of forms must be complete for each child. Please bring them with you on the first day of camp.
• Medication Administration Authorization Form
This form is not necessary for all campers; ONLY those campers that need prescription medication (routine medications, EpiPens, inhalers, etc.) while at camp need to complete this form. It must be signed by the prescribing physician.
